Always collect both glucose and insulin from the same fasting blood draw. Even mild stress or a short walk before the blood draw can acutely raise insulin levels through sympathetic activation, falsely elevating your HOMA-IR result.

The constant 22.5 in the HOMA-IR denominator was derived from the product of a 'normal' fasting glucose of 4.5 mmol/L and a 'normal' fasting insulin of 5 mU/L — values from healthy lean Oxford volunteers in the 1980s.
